Elderly Services Program

The Elderly Services Program provides support services to assist older adults to continue living independently in their own homes.

Eligibility

  • Warren County resident
  • 60 years of age or older
  • Have a need for services as determined by the case manager

There is no cost for case management or the initial phone call. Seniors with lower incomes receive services at no cost. Others are required to help pay for the cost of care, based on a sliding fee scale considering income and ongoing medical expenses. Home Delivered Meals are not included in the fee scale, but a donation is requested. 

ESP Provides:

  • Intake, Assessment and Case Management.
  • Home Delivered Meals delivers a nutritious lunch each weekday. Weekend lunches are also available.
  • Homemaking helps with grocery shopping, errands, laundry and light house-keeping.
  • Personal Care provides assistance with bathing, grooming, and dressing.
  • Adult Day Services has supervised programs providing a wide range of services in a stimulating environment. Transportation is also available.
  • Respite Care provides relief for the primary caregiver.
  • Electronic Monitoring Service provides an in-home monitoring system for older adults providing safety alerts.

  • Medical Transportation to doctors, hospitals and clinics in Warren County and bordering counties.
  • Minor Home Repair/Accessibility building ramps, installing grab bars and deadbolt locks.
  • Independent Living Assistance with finances, personal business and makes linkages to available services.A phone call to the ESP Intake and Assessment team starts the process to receive services. A case manager makes a home visit to develop a plan of care, personalized to individual needs.

  • Social Security Number
  • Medicare or Medicaid card number
  • Supplemental Health Insurance Information
  • Information regarding on-going medical expenses and monthly income ESP recipients are expected to apply for services and benefits for which they are eligible including Medicare, Medicaid, PASSPORT or private insurance.
